No.  Go to the website that Hideo posted and submit.  There is no fee.
I expect that the website is all you need.  If that is incorrect, an
update will be forthcoming.

Even if you never expect to use either band, REGISTER!  Doing so
prohibits UTC from deploying future PLC's within the 1km radius of
your station and further helps hams.

My personal opinion is that I don't expect you to receive a response
unless they want to contest your notification.  For that matter, this
may go into a bit bucket and never be seen by anyone.  As I have
indicated all along, this is mostly smoke and mirrors for the lower 48
and 472 users so I don't think anyone is going to spend much time
scrutinizing.

I filed this morning, as have several others.  I expect that WG2XIQ
will be QRV for only 30 more days.
